4) Items to Bring
Please arrive 15 minutes early for your first visit with your completed forms, photo ID, Insurance Cards & Co-pay. Please present all information required at time of check in. If you have had any recent blood work or testing that might be related to the problem you are being seen for, please have it faxed to our office in advance of your appointment at (860) 444-2015. If you have been treated for a chronic (recurrent) problem, please have your PCP or treating physician fax over all notes/documentation regarding past infections and treatments. We need to have this information in order to obtain prior authorizations from your insurance company for medication, further testing and/or surgery, if indicated.
Canceling an Appointment
There is a $45 fee charged to patients for missed appointments. If you need to cancel an appointment, kindly give at least a 24 hour notice. If you are unable to call during regular business hours, you may call our answering service at 860-325-4155.
